Responsibilities
Northwest Specialty Hospital, an extension of Northern Nevada Medical Center, brings expanded services to a new building located at 6225 Sharlands Ave. in Northwest Reno. The facility will offer acute rehabilitation and adult behavioral health services. Our acute rehabilitation services include occupational, speech, and physical therapy for adults recovering from stroke, injury, or another disabling condition. We offer advanced treatment for a wide range of conditions including stroke, brain injury, amputation, neurological disorders, major multiple traumas and spinal cord and back injuries. All patients will experience private rooms, have access to a newly renovated therapy gym, and personalized care from our qualified staff. The adult behavioral health services offered is an expansion to our current 50 and older program at NNMC. The new program will offer behavioral health services to all adults 18 years and older. The department is managed through a multidisciplinary team including physicians, nurses, CNAs, social workers, and therapists. Patients will experience an individualized care plan, inclusive of individual and family therapy, activities, medication management, and a 24-hour secure, compassionate environment. Learn more at: https://www.nnmc.com This Case Manager opportunity is part time at 24 hours per week, offers full benefits and a convenient dayshift schedule. The case manager is responsible for overseeing the case management process within the hospital, which includes the assessment, planning, intervention, monitoring and evaluation of the patient’s care from admission to discharge. Collaborates with all members of the healthcare team to ensure the level, efficacy and quality of care are appropriate and discharge planning is facilitated through continuous planning and suitable referrals. Provides consultation and education to facility leadership, clinical and financial personnel and medical staff members related to case management, utilization and discharge planning practices. Job duties/Responsibilities: Understands and communicates the mission, strategic goals and policies of the department and Hospital. Holds regular staff meetings and ensures those not in attendance receive the information Responsible for performing assessments on Medicare, Medicaid and high-risk patients to determine discharge planning needs, always documents assessments. Able to communicate with physician regarding discharge planning needs of the patient. Assists the physician to facilitate post-hospital care.
