Behavioral Hospital of Bellaire is a freestanding, 122 bed facility specializing in general adult psych, adolescents, geriatric, and women's services. The hospital is located in southwest Houston serving the Houston and surrounding communities. Behavioral Hospital of Bellaire is conveniently located between the Galleria and Meyer land shopping districts, and is only minutes away from the Houston Medical Center, Downtown, and Midtown. Our continuum of care includes inpatient and outpatient psychiatric services to stabilize symptoms of depression, anxiety, and other behavioral health disorders. Much more available online at: https://bhbhospital.com/ Behavioral Hospital of Bellaire is seeking a full-time Discharge Planner to provide case management and support and coordinate ongoing patient care and aftercare needs. Discharge Planner/ Case Manager Key Responsibilities Include: Communicate with referral source within the first 24 hours of admission. Ensure that all placement documentation is completed according to established timelines. Complete patient file audits to ensure compliance. Responsible for discharge plans including scheduling of appointments and communication with parents and significant family contacts. Inform patients and families on discharge planning options based on diagnoses, prognoses, resources, and preferences related to home care services. Document findings, discharge plans, and actions taken on medical record according to departmental guidelines; prepare reports and maintain records as required. Act as an educational resource and provide consultation to hospital personnel regarding discharge planning process and applicable federal, state, and local regulations; identify benefits, implications, limitations of continued care as appropriate. Qualifications
Education: Bachelor's Degree from an accredited college or university, in Social Work or a clinical related mental health field highly preferred; High School Degree or equivalent required.
Experience
Minimum of one (1) year experience in related disciplines with hospital experience strongly preferred. Additional Requirements: Successful completion of CPR and Handle With Care certification; certifications may be obtained during orientation.
