FPN Munster Medical Office Building 759 45th St. Munster, Indiana 46321 The Community Health Care Advocate (CHA) is an integral member of the care team at our FPN Munster OB/GYN office helping to bridge the gap between health care providers, community services providers, and individuals in the community. You will assist with care plan implementation, help develop care management strategies, and work with team members to provide linkages for various health and social needs of patients to reduce disparities, improve health status, and community health through outreach and health education.

WHAT YOU CAN EXPECT A day shift schedule (8A-5P) Schedule medical appointments and transportation, conducting reminder/confirmation phone calls, and completing referrals to any additional services. Coordinate with health care providers to report on patient progress and conference, if intervention needs to be modified or discontinued. Work with community agencies, social workers, health educators, peer navigators, and health care providers to link patients to needed preventative health services, to prevent illness, improve skills for self-management of chronic conditions, and promote healthy lifestyle behavior change. Accompany patient to appointment as needed; may be virtually; assist patient navigating the medical system. Play a consistent and active role in identifying project inefficiencies and finding collaborative solutions to the problems.
REQUIRED QUALIFICATIONS
High School Diploma/GED Community Health Worker (CHW) Certification within 1 year of hire PREFERRED QUALIFICATIONS 1 year of Medical Assisting experience in an OB outpatient setting TRAVEL IS REQUIRED: Up to 50% JOB RANGE: INCENTIVE:
