We're hiring a Licensed Practical/Vocational Nurse (LPN/LVN) to monitor a panel of remotely enrolled patients and move the ones who need clinical attention into care the same day. You will review incoming vitals and symptom data for patients with hypertension, heart failure, and diabetes, working alongside Cadence nurses and nurse practitioners to act on risk between visits. This role sits inside Cadence's Clinical Intelligence workflows, where alerts are surfaced continuously and licensed clinicians decide what happens next.
This is a full-time, remote position. The scheduled hours for this role are Monday through Friday from 9:00 AM to 6:00 PM Eastern time or 10:00 AM to 7:00 PM Eastern Time.
What You'll Do
• Monitor incoming vital signs, symptoms, and health indicators for a panel of patients enrolled in Cadence remote monitoring.
• Triage alerts and abnormal readings against established clinical criteria and identify patients who require immediate clinical intervention.
• Contact patients with out-of-range readings to gather additional clinical information and confirm symptom, medication, and adherence context.
• Coordinate with Cadence nurses and nurse practitioners to route flagged patients into the right intervention and close the loop on every follow-up.
