Behavioral Health Caseworker

We are looking for a Behavioural Health Caseworker to manage and coordinate care for members with behavioural health conditions and complex needs. You will serve as the primary contact for care teams, develop and implement care plans, and support members through transitions. Ideal candidates have experience in social services and strong assessment and communication skills.
