The Role
We're hiring an RN Manager, Alerts to lead and scale the nursing teams responsible for alert review and triage across our chronic care population. This role owns the clinical operations behind how Cadence responds to vitals that signal risk in patients with hypertension, heart failure, and diabetes, coordinating RN and LPN teams that act on those signals between visits. You will report to clinical operations leadership and partner with providers and care teams to keep alert response fast, accurate, and grounded in guideline-directed care.
What You'll Do
• Lead RN and LPN teams responsible for alert review and triage, with clear accountability for response time and clinical quality across the chronic care population.
• Coach and develop clinical staff, setting performance standards that hold triage decisions to guideline-directed care for hypertension, diabetes, and heart failure.
• Own staffing, scheduling, and workload distribution so alert volume is covered efficiently without compromising clinical judgment.
• Build and refine the triage and alert workflows that determine how quickly a concerning vital reaches the right clinician for action.
• Partner with cross functional teams to close the loop between surfaced risk and clinical intervention between visits.
• Analyze team performance data to identify bottlenecks, strengthen escalation pathways, and scale operations as patient volume grows.

What You Need
• Active, unencumbered compact RN license required.
• Minimum of 5 years of clinical experience as a Registered Nurse, including work in triage, remote patient monitoring, or chronic disease management for conditions such as hypertension, heart failure, and diabetes.
• Minimum of 3 years of nursing leadership experience with full supervisory responsibility for clinical teams, including hiring, performance management, coaching, and development in high-volume, queue-based, or remote care delivery environments.
• Track record of scaling clinical operations in a growth-stage or early-stage company.
• Proficiency with EHR systems and performance data tools, with the ability to work across platforms such as Athena, Epic, Zendesk, Notion, and G Suite.
• Fluency with AI-assisted clinical tools (such as Gemini, Open Evidence, or comparable platforms) and the ability to speak credibly with providers and patients about where AI supports care delivery and where human judgment remains non-negotiable
Compensation
The anticipated OTE range for this role is $110,000 - $140,000, which is based on role scope, level, and location. In addition to base compensation, this role may be eligible for incentive compensation as part of the overall total rewards package. This position is remote and based in the United
