Licensed Practical Nurse (LPN) Discharge Lounge

Unit Highlights The Discharge Hospitality Center (DHC) Licensed Practical Nurse (LPN) facilitates a safe, efficient, and patient-centered discharge process for patients awaiting departure from the hospital. The LPN provides individualized discharge education and reinforces instructions to ensure patients and caregivers understand medications, follow-up appointments, and post-discharge care. Responsibilities include completing accurate documentation, assessing patients for any additional care needs while in the lounge, coordinating safe and appropriate transportation home, and maintaining a safe, comfortable environment until discharge. The LPN collaborates closely with the interdisciplinary team to promote a seamless transition from hospital to home while supporting patient satisfaction and safe post discharge outcomes.   Full-time Monday-Friday   Key Responsibilities As a Clinical Nurse, you will provide skilled, compassionate care to patients and families across a spectrum of needs—from routine to highly complex. You will assess evolving patient conditions, identify actual and potential health concerns, and develop appropriate care plans. This position has a hiring range of USD $23.33 - USD $37.82 /Hr.
