Job Summary
The Assessment Referral Coordinator supports patient care by coordinating referrals, authorizations, and follow-up appointments in collaboration with physicians and other care team members. This role ensures timely scheduling, accurate documentation, and clear communication with patients to facilitate continued access to services and high-quality care. Essential Functions Coordinates patient referrals, authorizations, and appointments for specialty care, diagnostics, and follow-up services in accordance with provider orders. Communicates with patients and families to explain next steps, provide instructions, and ensure understanding of referral and appointment requirements. Obtains prior authorizations and referral approvals as needed, and accurately documents information in the electronic health record. Schedules appointments and follows up with external providers to confirm referral completion and collect consult notes. Responds to incoming calls and referral requests, triaging appropriately and ensuring timely processing. Maintains accurate and organized patient records, ensuring documentation is complete and submitted according to policy. Performs other duties as assigned. Maintains regular and reliable attendance. Complies with all policies and standards.
Qualifications
Associate Degree in Healthcare Administration or related field preferred 0-2 years of experience in a healthcare administrative or referral coordination role required Knowledge, Skills and Abilities Knowledge of referral processes, insurance requirements, and prior authorization procedures. Proficiency with electronic health record systems and scheduling software. Excellent verbal and written communication skills. Strong attention to detail and organizational skills. Ability to manage multiple tasks in a fast-paced environment. This position is not eligible for immigration sponsorship now or in the future.
