Field Care Coordinator

Become a part of our caring community   The Care Coach 1 assesses and evaluates member's needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Care Coach 1 work assignments are often straightforward and of moderate complexity. Reports to the Regional Care Coach Manager. Looking for motivated Care Coach in Sarasota county FLORIDA!! We are looking for dynamic case managers that enjoy making a difference in the lives of others! You must live in Sarasota county in Florida. This rewarding role allows you to spend time connecting with our members to ensure they receive the services they need. The Care Coach 1 employs a variety of strategies, approaches and techniques to support a member's optimal wellness state by coordinating services & resources. Identifies and resolves barriers that hinder effective care. Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through use of assessment, data, conversations with member, and active care planning. Understands own work area professional concepts/standards, regulations, strategies and operating standards. Work is managed and often guided by precedent and/or documented procedures/regulations/professional standards with some interpretation. The Care Coach 1 Must live in Sarasota county in Florida Visit Medicaid members in their homes, Assisted Living Facilities, and/or Long Term Care Facilities and other care settings – 75-90% local travel Assesses and evaluates member's needs and requirements in order to establish a member specific care plan Ensures members are receiving services in the least restrictive setting in order to achieve and/or maintain optimal well-being Planning and implementing interventions to meet those needs Coordinating services, and monitoring and evaluating the case management plan against the member's personal goals Guides members/families towards resources appropriate for their care Services are driven by facilitating interactions with other payer sources, providers, interdisciplinary teams and others involved in the member's care as appropriate and required by our comprehensive contract Use your skills to make an impact   Required Qualifications Must live in Sarasota county in Florida. Bachelor's degree in health and human services field 2 or more years of related experience Exceptional verbal/written skills Use of a variety of electronic information applications/software programs This role is part of Humana's Driver safety program and therefore requires an individual to have a valid state driver's license, reliable vehicle, and proof of personal vehicle liability insurance with at least 100,000/300,000/100,000 limits.

Previous experience with electronic case note documentation and experienced with documenting in multiple computer applications/systems Prior experience with Medicare & Medicaid recipients Experience with health promotion, coaching and wellness Knowledge of community health and social service agencies and additional community resources Bilingual Spanish/English     Florida Background Screening Requirements: Notice for candidates applying to this position from a Florida work location, including Florida home-based work location. Scheduled Weekly Hours 40 Pay Range The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
