 Responsibilities: Presbyterian Healthcare Services is seeking a dedicated Behavioral Health Therapist or l icensed Social Worker to serve as a key member of the interdisciplinary care team as Care Manager , providing comprehensive therapeutic services to individuals experiencing mental health and behavioral health challenges. In this impactful role, you will guide patients through assessment, treatment, crisis intervention, discharge planning, and recovery while partnering closely with physicians, nurses, case managers, and other healthcare professionals. As a Care Manager , you will play a vital role in helping patients achieve their treatment goals through individual counseling, group therapy, case management, and care coordination. You will advocate for patients throughout their hospitalization, ensuring access to appropriate services, facilitating smooth transitions of care, and supporting positive clinical outcomes. This position combines direct patient care, clinical expertise, utilization management, and multidisciplinary collaboration in a dynamic behavioral health setting. How you will support our community and the team. Conduct comprehensive psychosocial assessments and develop individualized treatment plans. Provide counseling, brief psychotherapy, case management, and therapeutic interventions for assigned patients. Facilitate educational, supportive, and therapeutic group sessions that promote recovery and wellness. Serve as the primary therapist for assigned patients, coordinating care and monitoring progress throughout treatment. Collaborate with patients, families, and the interdisciplinary team to support treatment goals and successful outcomes. Lead discharge planning activities, including family meetings, follow-up care coordination, appointment scheduling, and post-discharge support. Complete and maintain timely, accurate, and thorough clinical documentation in accordance with regulatory and organizational standards. Manage utilization review activities, including prior authorizations, concurrent reviews, appeals, and coordination with payers to ensure appropriate use of healthcare resources. Address patient complaints, adverse events, and risk management concerns through proper documentation and follow-up actions. Participate in quality improvement initiatives, peer reviews, audits, accreditation readiness activities, and clinical practice enhancements. Qualifications: Masters degree in Counseling, Social Work or Psychology . 3 years experience relevant to specific program needs and patient populations, prefer experience in psychiatric inpatient acute care setting.
