Position

Summary: Therapist - is responsible for planning, coordinating, documenting, and facilitating the patient intake process in accordance with the hospital’s established policies and procedures, including conducting comprehensive admission assessments, gathering clinical and psychosocial information, and coordinating with referral sources, patients, and families to ensure appropriate level-of-care placement; ensures all admission documentation is completed accurately and in compliance with hospital standards and regulatory requirements; collaborates with clinical staff to initiate the Master Treatment Plan at the time of admission and communicates relevant patient information to the treatment team to support continuity of care; provides crisis assessment as needed and works closely with physicians, nursing staff, and families to ensure a smooth, timely, and efficient admission process while promoting a positive patient and family experience.
