As a Community Health Evaluator, you will play a key role in advancing community health initiatives through data-driven evaluation, performance measurement, and continuous quality improvement. Partnering closely with community and organizational stakeholders, you will use both quantitative and qualitative methods to assess program effectiveness, strengthen data quality, and support evidence-based decision-making. In this role, you will help conduct community health assessments, evaluate the impact and reach of health programs and interventions, and contribute to the planning, implementation, reporting, and ongoing improvement of community health strategies. Your work will directly support community health priorities and drive meaningful outcomes that improve the lives of those we serve.

 Responsibilities: Plan, implement, and maintain monitoring and evaluation systems, including data collection tools, databases, and performance measurement processes. Design and coordinate qualitative and quantitative data collection, analysis, and reporting activities. Conduct evaluations using public health evaluation frameworks to measure program effectiveness, outcomes, and community impact. Support community health assessments and the planning, implementation, evaluation, and reporting of community health initiatives. Develop evaluation plans, research protocols, and reporting requirements to support grant-funded programs. Prepare and submit regulatory, evaluation, progress, and outcome reports for funders and stakeholders. Collaborate with community health leadership, healthcare partners, community organizations, and tribal partners to develop shared evaluation measures and performance indicators. Provide training and technical assistance on health assessments, program planning, logic models, quality improvement, program monitoring, and evaluation practices. Analyze and synthesize local, regional, and national data to communicate findings, outcomes, and recommendations. Build and maintain collaborative relationships with community, governmental, internal, and tribal stakeholders. Advance community health priorities and support collective impact initiatives identified through community health needs assessments and implementation plans. Promote the adoption of evidence-based, population health approaches across the organization. Perform other duties as assigned.

Qualifications

Minimum Qualifications Master’s degree in public health, Epidemiology, Public Administration, Health Education, Health Administration, Statistics, or a related field. Minimum of one (1) year of experience in public health, including data collection, data analysis, and program evaluation activities.

Preferred Qualifications

Master’s degree in Public Health, Epidemiology, Public Administration, Health Education, Health Administration, Statistics, or a related field. Two to three (2-3) years of experience in public health, including data collection, data analysis, program evaluation, and performance measurement.
