Utilizing lived experience with recovery and/or reentry, the Specialist offers emotional support, guidance, and resource connection to promote successful reintegration, reduce recidivism, and improve overall well-being. The Peer Support Specialist will coordinate with health service providers, the Clinical Case Manager, the discharge planner, and reentry staff to provide outreach, intake, and wrap-around services for individuals participating in the MAT program. They must be trained in peer counseling, life skills, and the impact of reentry on individuals. The PSS must be qualified and trained to provide effective diversion and/or reentry services and maintain the confidentiality of program participants’ medical and justice involvement history. Hours are from 8am-5pm Responsibilities: Provide one-on-one peer support to individuals involved in reentry and jail diversion programs, sharing personal recovery experiences to inspire hope and motivation. Foster a safe, non-judgmental and culturally sensitive environment for clients. Facilitate peer support groups and educational workshops focused on recovery, coping skills, and community resources. Collaborate with multidisciplinary teams including case managers, social workers, probation/parole officers, and healthcare providers to coordinate care. Help develop individualized reentry plans supporting clients’ goals and recovery journeys. Maintain accurate documentation of support activities and client progress according to program requirements. Complete intakes and report compliance.
