Primary Responsibilities
Manage a dedicated population of Bleeding Disorders and Alpha1 patients through high touch inbound and outbound case management, including monthly refill coordination and ongoing clinical assessments aligned with performance guarantee standards. Provide comprehensive onboarding for new patients, ensuring education, therapy initiation, and continuity of care through a high touch service model. Serve as a central point of contact and liaison between patients, pharmacists, prescribers, nurses, sales, supply chain, and infusion teams to ensure exceptional, coordinated patient care. Support scheduling and coordination of treatments in the Ambulatory Infusion Suite and/or other outpatient care settings. Patient Interaction & Problem Resolution[FS1] Utilize active listening, root cause analysis, and critical thinking to identify patient needs, uncover underlying issues, and implement effective solutions. Strive for first call resolution by providing accurate, timely, and patient centered responses to inquiries. Demonstrate awareness of internal and external factors affecting patient experience, including insurance plan changes, call routing updates, supply disruptions, or weather-related events. Enter and process prescriptions from intake through order completion, verifying patient demographics, insurance, and clinical information for accuracy and completeness. Type, review, and prioritize prescriptions based on urgency; assess data integrity; monitor claims processing status; and identify root causes of claim rejections. Process prescription refills, interpret therapy specific supply needs, and coordinate timely medication delivery. Required Qualifications
Active and unrestricted Pharmacy Technician licenses where required by state law: If located in Nevada, current active and unrestricted
