About the Role

We are   seeking   a compassionate and patient-focused   Part-Time Nurse Case Manager   to support patients, healthcare providers, and clinic partners through a specialized Patient Support Program (PSP).   As a key member of the care team, you will help patients navigate their treatment journey by providing education,   care   coordination, and ongoing support. You will collaborate with physicians, clinics, pharmacies, and internal stakeholders to   facilitate   timely   access to treatment and help deliver an exceptional patient experience.   This is a part-time position requiring approximately   2.5 days per week . Scheduled hours must fall within the program's operating   hours of   Monday through Friday, 8:00 a.m. to 8:00 p.m. EST .

What You'll Do

Serve as a primary point of contact for patients enrolled in the Patient Support Program. Provide education   regarding   prescribed therapies, treatment expectations, medication administration, and adherence strategies.   Support patient enrollment and onboarding activities.   Coordinate care and communicate with physicians, clinic staff, specialty pharmacies, and other healthcare professionals.   Assist   patients in navigating access to treatment and reimbursement processes.   Conduct patient assessments and follow-up activities as required by the program. Monitor patient progress and provide ongoing support throughout the treatment journey. Maintain   accurate   and   timely   documentation within program systems. Identify   and escalate clinical or operational concerns as   appropriate . Contribute to quality improvement initiatives and program success measures. Build and   maintain   positive working relationships with healthcare providers and clinic partners.

Basic Requirements

Active and unrestricted nursing license in   Alberta and/or British Columbia   (RN, RPN/LPN, or equivalent nursing designation).   Diploma, degree, or equivalent combination of education and experience consistent with the   P1 Professional Career Framework .   Strong verbal and written communication skills.   Proficiency   using Microsoft Office and electronic documentation systems.   Ability to work approximately   2.5 days per week   within program operating hours of   8:00 a.m. to 8:00 p.m. EST, Monday through Friday .   Ability to travel to clinics and healthcare sites throughout Alberta and British Columbia as   required .   Excellent organizational skills and attention to detail.     Preferred Skills & Experience   Experience working within a   Patient Support Program (PSP) .   Prior   Nurse   Case Management experience.   Experience supporting patients receiving specialty medications or biologic therapies.   Knowledge of reimbursement, patient access, or specialty pharmacy processes.   Experience providing patient education and clinical support.   Experience collaborating with physicians, specialty clinics, and multidisciplinary healthcare teams.   Familiarity with chronic disease management and   treatment   adherence programs.   Additional   active nursing licenses in Canadian provinces.     Travel, Work Environment & Physical Requirements   Primarily remote/home-based role with travel as   required .   Travel to clinics, healthcare facilities, and stakeholder meetings throughout Alberta and
