Senior Manager, Care Management - Transitions of Care

A bit about this role:  Devoted Health’s long-term vision is to create a virtual "blue zone" for its members, where they can live longer, healthier lives by receiving the right care, in the right place, at the right time, in a proactive and coordinated way.  When the traditional/ ‘status quo’ health system fails to provide, Devoted members can rely on Devoted Medical, an innovative, world-class care delivery system whose primary directive is to treat each member like family.   Transitions of Care (TOC) is one of Devoted Medical’s largest clinical programs,  responsible for ensuring that thousands of Devoted members return home safely after hospitalization.  From admission through 30 days post-discharge, our highly skilled, virtual interdisciplinary teams deliver personalized and comprehensive ‘wrap around care’ to break down the gamut of clinical, social & logistical barriers to post-discharge stability.  This member-centric approach has already yielded substantial, sustained reductions in post-discharge complications and readmissions, and we’re continuously evolving our care model to keep more members safe at home.  That’s where you come in.  We are looking for a seasoned, conscientious, and transformational clinical leader and thought partner, a scrappy, empathetic, determined individual with a proven track record in leading high performing clinical teams.  Partnering closely with TOC’s clinical & operational leaders, the Senior Manager will be accountable for a significant frontline workforce (~60-70 frontline RNs) to provide excellent interdisciplinary care in the 30-day window after patients leave the hospital. The Senior Manager will develop and inspire a team of managers to lead engaged, quality-oriented teams that obsess over caring for patients like family and help them stay healthy at home. Required skills and experience: (4-5 bullets recommended max*) 5+ years of experience leading high performing clinical teams in a transitional care and/or care management setting, ideally across a virtual and/or geographically dispersed workforce 3+ years of experience in coaching and developing other leaders/managers (i.e. past manager-of-managers) Active RN license in good standing, and/or BSN or equivalent related degree, master’s preferred Experience in post-acute medicine, nursing and case management processes, internal medicine, urgent care, emergency room, hospital medicine, and/or geriatrics Experience managing and/or working in an interdisciplinary care team setting, along with a strong conviction for interdisciplinary care in general Extensive experience as an enthusiastic change agent/transformational leader Desired skills and experience: (1-2 bullets max*) Compassionate, service-oriented leader that rolls up their sleeves, shows their work, seeks feedback & builds consensus Curious & collaborative operations-minded problem-solver who knows how to recognize process deficiencies, recommend improvements and implement solutions Energized by a fast-paced, rapidly growing organization, with the agility, resilience, and a growth mindset to embrace change and rapid cycles of quality improvement Deeply empathetic and humanistic- you develop engaged teams that have a deep culture of service to members/patients Excellent communication skills, someone who thoughtfully adapts their style based on audience and nature of the message in both written and verbal contexts Certification in Case Management a plus!
