Approximately 10% of all dialysis patients reside in a SNF and are transported to dialysis three times a week placing a significant burden on patients and adding significant avoidable costs to the health care system. SNF dialysis patients receive dialysis treatment onsite at the Skilled Nursing Facility in which they reside. The RDs likely to have a starting caseload of around 110, but this has the potential to grow. The RD can work a hybrid schedule, but must be located in the Phoenix metro area. The number of days on-site may vary. It is likely to be about two days per week from home and three days on-site meeting with patients at our Arizona Skilled Nursing Facility Dialysis locations. 1+ year of CLINICAL Registered Dietitian experience State licensure required if licensure is available in the state where the facility is located Bachelor's degree in Nutrition, Dietetics, or similar area required Ability to work flexible schedules, possibly for multiple facilities and travel when needed After hire and training, successful completion of the Competency Assessment for Renal Dietitians (CARD) with a score of at least 80% required Intermediate computer skills and proficiency with MS Word, Excel, and Outlook required as well as functional proficiency with DaVita specific applications within 60 days DaVita is a clinical leader!
