RN Hospice Registered Nurse (PRN)

Registered Nurse – Polk County Territory Compassionate Care Hospice (CCH) , an Amedisys Company   **This position is based in Lakeland, FL and requires travel to assigned area(s) in Polk County  Schedule: Flex - min. 10hrs./wk.   At CCH, we go above and beyond—not only for our patients but for each other.  Compassion, collaboration, and commitment are at the heart of everything we do.

On average our orientation for an RN is 7-9 weeks, however is individualized according to your needs and experience.

Performs initial nursing assessment on new admissions. Completes , coordinates, and revises the total plan of care and maintains continuity of patient care by collaborating with appropriate staff. Regularly communicates patient progress to the clinical manager, physician, and care team. Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies. Records pain/symptom management changes/outcomes as appropriate. Educates families on patient condition, needs and care. Makes referrals to other hospice care disciplines as needed , or documents rationale for not doing so. Performs on-call responsibilities and on-call services to patients/families as assigned. Performs other duties as assigned.

Required: Current RN license  in the state(s) where you will work Minimum 1 year of clinical RN experience in an acute healthcare setting Current CPR certification Preferred: Hospice or home health experience Strong communication and interpersonal skills Ability to work independently and manage multiple priorities Comfortable with daily travel between facilities Proficient in electronic medical record (EMR) systems   Ready to take the next step?
