Maximus is currently hiring a Field Clinical Assessment Coordinator to join our Outreach & Assessment team. Our Field Clinical Assessment Coordinators play a critical role in helping individuals explore community-based options by conducting person-centered outreach interviews, transitional assessments, and clinical evaluations that identify their needs, risks, strengths, and preferences. This position requires travel up to 85% throughout Cook County and the greater Chicago metropolitan area, with occasional travel outside the region based on business needs. About the program: The Illinois Outreach & Assessment (O&A) Program is a centralized, conflict‑free initiative operated by Maximus to support individuals identified under the Williams and Colbert Consent Decrees. The program provides person‑centered outreach and transitional assessments to help individuals explore and understand potential community transition options. O&A staff engage individuals directly, complete required outreach interviews and transitional assessments in accordance with state protocols, and collaborate with state agencies and community partners to ensure timely, accurate, and high‑quality assessment outcomes. Essential Duties and Responsibilities: - Complete medical and/or behavioral health assessments that are typically routine using standardized tools and criteria in accordance with contract requirements; escalate complex or non-standard cases that fall outside defined protocols. - Conduct on-site, video, and/or telephonic assessments based on program requirements to determine an individual's needs for services and supports, eligibility, and level of care based on established guidelines, seeking guidance from senior staff as needed. - Perform all job duties in compliance with Person First standards, HIPAA guidelines, and company confidentiality policies; ensure accurate and consistent documentation of assessment outcomes. - Travel to client, consumer or program locations to perform in-person assessments exceeding 50% of the workweek in support of program delivery.

- Conduct in-person, on-site outreach interviews and transitional assessments using person-centered and motivational interviewing techniques - Educate individuals about their rights, the Outreach & Assessment process, and what to expect in next steps - Accurately document assessment findings, outcomes, and required data elements in program systems - Maintain professionalism and strong customer service when working with individuals with complex medical, behavioral health, and social needs - Traveling as required based on program contract requirements. - Perform other related duties as assigned.

Minimum Requirements

- High school diploma or GED required and 2+ years of relevant professional experience required, or equivalent combination of education and experience. - Travel
