St. John Family Medicine 8345 Wicker Ave St John, Indiana 46373 The Social Worker, LCSW provides individuals, families, and groups with the psychosocial support needed to cope with chronic, acute or terminal illnesses. This position advises, educates, and counsels patients and their loved ones based on their needs and concerns. In addition, the Social Worker, LCSW provides crisis intervention, refers patients to appropriate services, and assists patients and their loved ones in understanding the implications and complexities of the situation to ensure the patient and their loved ones receive continuity of care and services through resource management.

WHAT YOU CAN EXPECT Schedule: Monday - Friday, Day shift, 40 hours per week (no weekends or company recognized holidays) Collaborate with multidisciplinary teams to evaluate and assess patients’ medical and physical needs. Refer patients to appropriate services if patients are identified with depression, anxiety, grief or mental illness. Utilize consultation data and social work experience to plan and coordinate client or patient care and rehabilitation, following through to ensure service efficacy. Identify environmental impediments to patient progress through interviews and review of patient records. Assist patients and their caregivers in identifying goals of care and assists with unmet needs through linkages to community resources. Assist in crisis intervention. Document patients’ electronic medical records in a clear, concise, accurate, and timely manner.
QUALIFICATIONS
