Service Coordinator-CHC

We are looking for a Service Coordinator to support Medicaid participants in Chester, Bucks, Montgomery, Delaware, and Philadelphia counties. You will coordinate long-term services and support, conduct face-to-face assessments, and manage participant needs. Ideal candidates have experience in social services or healthcare and strong communication skills.
