Required Qualifications: * Doctor of Medicine (M.D.), Doctor of Osteopathy (D.O.), or M.B.B.S * Board Certified or Board Eligible in Internal Medicine or Family Medicine * Active and unrestricted Arizona medical license or ability to obtain prior to start * Current and unrestricted DEA certificate or
