Essential Duties & Responsibilities Deliver High-Quality Primary Care
Perform comprehensive health assessments and develop individualized treatment plans for residents in skilled nursing, assisted living, and independent living communities. Diagnose, treat, and manage acute and chronic medical conditions with a focus on preventative care and long-term wellness. Provide ongoing care management that improves quality of life while helping residents remain healthier and more independent. Build Meaningful Patient Relationships Conduct regular facility rounds and develop strong relationships with residents, families, and care teams. Monitor changes in health status and proactively intervene to prevent complications and unnecessary hospitalizations. Educate residents and caregivers on disease management, preventative care, and wellness strategies. Collaborate Across the Care Continuum Partner closely with physicians, clinical staff, facility leadership, specialists, and family members to coordinate comprehensive care plans. Serve as a trusted clinical resource within the facility and contribute to interdisciplinary care discussions.
Drive Clinical Excellence
Support initiatives focused on improving patient outcomes, enhancing quality measures, and reducing avoidable hospital readmissions. Maintain accurate and timely documentation within the electronic medical record (EMR). Ensure compliance with applicable regulatory, clinical, and organizational standards.
