About this role
Become a part of our caring community As a Telephonic Registered Nurse Care Manager, you will support patients by coordinating their care, assessing their healthcare needs, and helping ensure they receive the appropriate services and resources. You will be working fully remote and communicating with patients, caregivers, providers, and other members of the healthcare team. Your clinical experience will help you identify patient needs, provide education, and support safe care planning. You will manage care for patients with a variety of healthcare needs. You will complete telephonic assessments, review patient information, identify potential barriers to care, and help develop appropriate care plans based on each patient's individual needs. You will communicate with patients and caregivers to provide education, answer questions, and help them understand their plan of care. You will also collaborate with physicians, home health clinicians, hospitals, and other healthcare professionals to help ensure continuity of care. Your responsibilities may include reviewing clinical documentation, monitoring patient progress, identifying changes in condition, coordinating services, and helping patients access available healthcare resources. You will use your clinical judgment and experience to recognize potential concerns and escalate them. You will be expected to document patient interactions accurately and timely while maintaining confidentiality and following all applicable policies and regulatory requirements. Use your skills to make an impact Required Qualifications An active and unrestricted Registered Nurse license. 3–5 years of clinical acute care experience . at least 2 years of home health experience . Experience reviewing clinical information and using your nursing judgment to identify patient needs. Experience working with electronic medical records and other computer-based healthcare systems.
Preferred Qualifications
Previous experience in care management, case management, utilization management, or telephonic nursing. Experience coordinating care between hospitals, physicians, home health agencies, and other healthcare providers. Experience working with patients who have chronic or complex medical conditions. Strong knowledge of healthcare resources, care coordination, and transitions of care. Experience working in a remote or telephonic clinical environment. Scheduled Weekly Hours 40 Pay Range The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc. $71,100 - $97,800 per year This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
