Responsibilities
Foundations Minnesota is a member of Foundations Recovery Network, a nationwide network of addiction care with more than 20 years of providing evidence-based, patient-centered treatment for substance use and co-occurring mental health conditions. We are seeking a compassionate and dynamic Recovery Care Coordinator - CPRS (Certified Peer Recovery Specialist) to join our dedicated team. The Recovery Care Coordinator provides recovery-focused support, treatment coordination, and client engagement services to individuals receiving substance use disorder and behavioral health treatment services. Drawing upon lived experience and peer support principles when applicable, this position helps clients engage in treatment, overcome barriers to recovery, develop community supports, and successfully transition across levels of care. This is a salaried, full-time, benefits-eligible position at our Eden Prairie Outpatient Treatment Center, with a morning schedule Monday through Friday. Duties
Establish supportive, recovery-oriented relationships with clients to enhance treatment engagement and retention. Provide peer-based encouragement, mentoring, advocacy, and recovery support using lived experience when appropriate. Assist clients in identifying strengths, recovery goals, and personal wellness strategies. Meet individually with clients to assess barriers to treatment participation and recovery success. Conduct outreach and follow-up activities to support attendance, participation, and continuity in treatment services. Facilitate client engagement in recovery support meetings, peer support services, and community-based recovery activities. Qualifications
Qualifications Education: A bachelor's degree in behavioral science or related field, or other qualifying credentials.
Experience
2+ years of treatment coordination experience. Experience collaborating with individuals with substance use disorders and knowledge of community resources License: Must hold a CPRS Certification. Additional Requirements: Knows and understands Minnesota Statutes Chapters 245G, 260E (Child Maltreatment), and 245A, 626.557 and 626.5572 (Vulnerable Adults). Thorough knowledge of CARF, state and federal regulatory requirements.
